CALIFORNIA LAW (SB 746) PROHIBITS PERSONS UNDER THE AGE OF 18 FROM USING AN ULTRAVIOLET TANNING DEVICE.

THE FILANTE TANNING FACILITY ACT OF 1988 STATES: Failure to use eye protection provided to customers by the sun tanning facility may result in permanent damage to the eyes. Overexposure to ultraviolet light causes burns. Repeated exposure may result in premature aging of the skin and skin cancer. Abnormal skin sensitivity or burning may be caused by reactions of ultraviolet light to certain foods; cosmetics; or medications, including, but not limited to the following: tranquilizers, diuretics, antibiotics, high blood pressure medication, or birth control pills. Any person taking a prescription or over-the-counter medication should consult a physician or pharmacist before using an ultraviolet suntanning device. Any person with skin that burns easily should avoid an ultraviolet suntanning device. Any person with a family history or past medical history of skin cancer should avoid an ultraviolet suntanning device. I have read the above and have been provided with eye protection and proper instruction for the use of such eye protection. I agree to use protective eyewear.

HYDRATION STATION: Any person suffering from heart/respiratory problems, high blood pressure, kidney disorders, nervous conditions (e.g. epilepsy), are pregnant, are breastfeeding, and/or are allergic to shellfish, seaweed, algae and/or other products from the sea should consult a physician before using the Hydration Station. I REPRESENT THAT I UNDERSTAND THE NATURE OF THIS PROCESS AND THAT I DO NOT HAVE ANY MEDICAL CONDITIONS OR ANY OTHER REASON OR CONDITION THAT WOULD PROHIBIT ME FROM RECEIVING HYDRATION STATION TREATMENTS. I understand individual results may vary. I have been given instructions for the proper use of the equipment and I will use it at my own risk. I recognize that using the hydration station will lead to perspiration, highlighting the importance of staying properly hydrated beforehand.

Legal Disclaimer Regarding Red Light Therapy: Red light therapy utilizes natural red light in the 630 nanometer range and natural infrared in the 850 nm range of the light spectrum. These natural spectrums of light offer a range of documented therapeutic benefits, including but not limited to cosmetic skin enhancements, wound healing, muscle recovery, and pain management. It is crucial to understand that the use of red light therapy is not intended to diagnose, treat, cure, or prevent any disease. Additionally, the therapeutic benefits of red light therapy have not undergone evaluation by the FDA.

SUNLESS TAN: Spray tanning is accomplished by the application of a solution containing the active ingredient: DHA (Dihydroxyacetone). DHA has been FDA approved for cosmetic use. Any person suffering from respiratory problems, is pregnant or nursing, and/or is allergic to DHA should consult a physician before using a spray tan. All skin types are different, providing accurate information regarding your skin type, allergies, or other conditions is essential to achieving a positive spray tan result. The FDA has advised that all users of DHA spray tanning services avoid inhaling or ingesting DHA. When using all-over sunless tanning sprays containing DHA it may be difficult to avoid exposure to sensitive areas including eyes, lips, mucous membranes or internally. The FDA, along with California Tanning Salons, Inc., recommends you take the following protective measures during your sunless tanning session: use of protective undergarments, use protective eyewear, use nose filters, and seal lips with lip balm. I REPRESENT THAT I UNDERSTAND THE NATURE OF THIS PROCESS AND THAT I DO NOT HAVE ANY MEDICAL CONDITIONS OR ANY OTHER REASON, OR CONDITION, THAT WOULD PROHIBIT ME FROM RECEIVING SUNLESS TANNING. I have been informed by my sales consultant that a Sunless Spray Kit containing additional eye, mouth and nose protection is available for purchase should I elect to use such a kit.


Having been advised and fully informed by California Tanning Salons Inc., DBA Cop-A-Tan and Sunkissed Tanning concerning the nature of the suntanning, sunless sprays, hydration station, and red light therapy treatments proposed to be administered, including the effects of such process and the possible risk and consequences of exposure to the equipment used in such process, I hereby authorize and direct them to administer the suntanning, sunless sprays, hydration station, and red light therapy treatments proposed to be administered. I hereby confirm that no assurance has been made to me regarding the results of the suntanning, sunless sprays, hydration station, and red light therapy treatments and I HEREBY ACKNOWLEDGE AND AGREE TO RELEASE, DEFEND, INDEMNIFY AND HOLD HARMLESS CALIFORNIA TANNING SALONS INC., DBA COP-A-TAN AND SUNKISSED TANNING, ITS PRINCIPALS, EMPLOYEES AND AGENTS, AND TO ASSUME FULL RESPONSIBILITY FOR ANY LOSS OR DAMAGE AND FOR ANY CLAIM, LAWSUIT, OR DEMAND FOR LOSS OR DAMAGE, THAT MAY OCCUR TO ME. 

I fully understand that the administration of the suntanning, sunless sprays, hydration station, and red light therapy treatments proposed to which I am consenting, including exposure, is not an exact science, and that no guarantees have been made regarding the process including possible skin reaction, side effects, allergic reaction, or other possible complications. It is understood that this CONSENT is being given in advance of any administration of the process, and is being given by me voluntarily, to induce them to exercise their best judgment as to the requirement of administering the suntanning, sunless sprays, hydration station, and red light therapy treatment process and procedures.  ALL SALES ARE FINAL. California Tanning Salons, Inc. is not responsible for lost or stolen articles.

My signature below constitutes my acknowledgement that (1) I have read, understand and fully agree to the foregoing CONSENT, (2) the proposed suntanning, sunless sprays, hydration station, and red light therapy treatments proposed process has been satisfactorily explained to me and I have all the information that I desire, and (3) I hereby give authorization and consent.

I have read the instructions for proper use of the sun tanning facility and do so at my own risk. I hereby release the owners, operators, or manufacturers from any damage or harm that I might incur due to my use of the facilities.
